A dad from Stirling says he's "angry and upset" at NHS Forth Valley.
Grant Polley took his 10 month old daughter, Brooke, to A&E - after she hurt her arm.
Doctors did not refer her for an X-ray on two separate occasions, but a third trip to Forth Valley Royal found Brooke had a double fracture in her left arm.
NHS Forth Valley said:
"We are sorry to learn that this family are unhappy with the care provided to their daughter.
"We would encourage them to contact the Patient Relations Department at Forth Valley Royal Hospital so they can look into the issues raised."
